LGBTQI Inpatient Survey

Thank you for your interest in the LGBTQQI [Lesbian, Gay, Bisexual, Transgendered, Queer, Questioning & Intersex] Inpatient Survey. The survey is self administered and ANONYMOUS .

The purpose of the survey is to gather information from LGBTQQI-identified individuals who have had the most recent experience in an Inpatient or Residential Facility in Washington State for substance use or chemical dependency treatment. We hope that with this information, we can advocate for culturally competent training to inpatient facilities in Washington State to better provide services to LGBTQQI clients, and possibly assist in advocating for a LGBTQQI-specific inpatient facility(ies) in Washington State.
